- FAILURE TO USE EYE PROTECTION provided to the customer by the tanning facility may result in damage to the eyes.
- OVEREXPOSURE to ultraviolet light causes burns.
- REPEATED OVEREXPOSURE may result in premature aging of the skin and skin cancer.
- Abnormal skin sensitivity or burning may be caused by reactions of ultraviolet light to certain: foods, cosmetics, or medications such as: tranquilizers, diuretics, antibiotics, high blood pressure medicines, birth control pills.
- Any person taking a prescription or over-the-counter drug should consult a physician before using a tanning device.
- A person with skin that always burns easily and never tans should avoid a tanning device.
- A person with a family or past medical history of skin cancer should avoid a tanning device.
No guarantees of specific results are offered or implied. You agree that you understand the risks involved and are proceeding at your own risk. You hereby agree that you have been properly informed and agree to release Sol Tanning, its employees, operators, successors, and manufacturers from any damages that you might incur at any Sol Tanning facility.
You understand and agree that you are prohibited from tanning more than once within 24 hours.
I have read the contents of this consent form carefully and state that I am not aware of any medical condition or other reason that would prohibit me from tanning. I understand that I will not be allowed to exceed the maximum allowable time on the tanning device. I understand that results vary, and that no guarantees of specific results are offered or implied. I understand the risks involved and am proceeding at my own risk. I hereby agree that I have been properly informed and agree to release Sol Tanning Centers, LTD, its employees, operators, successors, partners, investors, and manufacturers from any damages that I might incur due to the use of any Sol Tanning facility.
